Please enclose a copy of your packing slip along with your phone number. 

For your protection, please use Insured USPS or UPS for return shipment.

A credit for the price of the item and applicable taxes will be applied to your credit card.

Shipping and handling charges previously billed and return shipping costs are not covered under the terms of this

warrenty.
